New Patient Registration
Patients must live within the practice boundary to be eligible for registration.
We are unable to keep patients on our list who move out of the practices catchment area.
You will need to complete a registration form to register at the surgery. Please note that all under 18’s registrations need a copy of your birth certificate.
You need to complete a health questionnaire. This is so we have all the important information on your past medical history.
We will ask the appropriate authority to send your medical records to us. This will take at least 2-6 weeks.

Assignment to an Accountable GP
We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone. Although, every patient has a “Named Accountable GP”.
You can ask for a particular GP to be your listed doctor and wherever possible we try to accommodate this.
Furthermore, we try to ensure you can have continuity of care. We do this by providing appointments wherever possible with the GP of your choice. But, there may be circumstances where this is not possible.
