Appointments are available from Monday to Friday with a Diabetic Nurse.
All patients with diabetes will be offered an annual review appointment. Patients are required to see a Health Care Assistant (HCA) prior to their annual review with a Diabetic Nurse. The HCA will ask you a number of health-related questions, take a blood test, measure your height and weight, check your blood pressure and will ask for a urine sample (bottles are available to collect from the reception desk). You will also be asked to see the Diabetic Nurse in two weeks’ time following the date of your assessment.
During your appointment with the Diabetic Nurse, you will have the opportunity to talk about any problems you may have and to discuss your individualised treatment plan. The Diabetic Nurse will perform a foot risk assessment and agree a plan of care with you, including subsequent appointments.
Patients are able to contact the Surgery if they have any diabetic related queries in between planned appointments; the Diabetic Nursing Team endeavor to answer queries with either a face-to-face review or via a telephone consultation. GP appointments are also available, if required.
